I, on behalf of myself, my child/children, acknowledge that myself or my child/children will be participating in classes, clinics, and or other events in the sport of All Star Cheerleading, All Star Dance or Tumbling or any other event hosted by Force Elite Academy (FEA).

I understand the inherit risk associated with the sports activities. I agree not to hold Force Elite Academy responsible, nor negligent if I or my child were to be injured during class, team practice, event or open gym within any FEA facility.

I understand I can not film or take photography of my child or other child and adults while taking class within FEA facility (Child Protection Act) – more detailed policy can be found on our website.
