I, on behalf of myself, my child/children, acknowledge that myself or my child/children will be participating in classes, clinics, competitions and or other events in the sport of All Star Cheerleading, All Star Dance or Tumbling or any other event hosted by Force Elite Academy (FEA).
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of cheerleading, tumbling, stunting, acrobatics, and dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of action of any kind, including any and all claims of negligence, arising as a result of such activity from with liability could accrue to Force Elite Academy, its officers, agents, employees, instructors, owners and all affiliated entities.
I hereby agree to release Force Elite Academy and hold FEA harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in cheerleading, tumbling, stunting, acrobatics and dance on behalf of myself or my child/children as participants. I am aware that this is a release of liability and acknowledgment of my voluntary and knowing assumption of risk of injury. I have signed this document voluntarily and my own free will in exchange for the privilege of participation. If I am a minor, my parent and/or legal guardian has signed this document releasing FEA from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risk of injury inherent to this activity.
Furthermore, I agree that myself and/or my child/children must follow all FEA rules to participate in any activity. Failure to follow such rules, as well as any other admonitions by FEA Staff, will result in myself and/or my child/children being removed from the program with no refund of any amounts paid. Safety of all participants is of the utmost importance and paramount to proper Cheerleading, Dance and Tumbling instruction.

Participants will, at all times listen to FEA coaching staff instructions and follow all such instructions.

Participants will be mindful of others in the training environment and positively contribute to a healthy & supportive culture.

Participants must be in good health and free from any adverse medical conditions.

FEA asks that pregnant women, participants with pre-existing health issues or wearing “hard casts” do not participate in any Recreational Activities.
I understand that myself and/or my child/children are allowed to participate in the activity of Cheerleading, Dance or Tumbling only in consideration of (among other things) my agreeing to the provisions of this Release and Waiver, I, for myself and my spouse, my child/children, my parents, my heirs, executors, administrators, assigns, and anyone who may be representing me.
I understand that in the event of student/participant accident resulting in injury that FEA will contact the releasing party and/or emergency contact in the first instance. I hereby allow FEA to call an Ambulance on my behalf or behalf of my child in case of an emergency or if FEA cannot contact myself or my emergency contact. I understand that in the instance that an ambulance is called on my behalf that I am liable for the costs associated with the call out and will be appropriately covered for Ambulance prior to initiating activities with FEA.
IMPORTANT: The change to your legal rights set out in this document do not apply if death or injury is due to gross negligence by Force Elite Academy (FEA). Also see section 23(3)(b) & regulation 5 & of the Australian Consumer Law & Fair Trading Act & Regulations 2012. You do not have to agree to exclude, restrict or modify or waive any of your rights against FEA but FEA may refuse to allow you to participate in the Recreational Activities.
I HEREBY RELEASE, HOLD HARMLESS, DISCHARGE FOREVER, AND AGREE NOT TO SUE Force Elite Academy (FEA), its Foundation, subsidiaries, affiliates, employees and/or volunteers FROM ALL LIABILITY FOR ANY AND ALL LOSS, DAMAGE, INJURY TO PROPERTY, PERSONAL INJURY, DEATH OR OTHERWISE, arising from or in any way connected to the student/s listed in this document, participation in the sport of Cheerleading, Dance or Tumbling or any other Events hosted by FEA, whether it arises upon any theory of strict liability or otherwise.
I HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS Force Elite Academy (FEA), its subsidiaries, affiliates, employees and/or volunteers from any and all claims, including attorney’s fee and costs, arising from or in any way connected to to the student/s listed in this document, participation in the sport of Cheerleading, Dance or Tumbling or any other Events hosted by FEA.
I, AS THE RELEASING PARTY of myself or my child acknowledge and understand the inherit risk associated with the sports activities of cheerleading, tumbling, dance & any event associated with these activities. I willingly commit myself and/or my child to participate in the activities of Cheerleading, Tumbling & Dance. Knowing all risks of minor or major injury, concussion, paralysis or loss of life however low the chances may be. I agree not to hold Force Elite Academy responsible, nor negligent if I or my child were to be injured during class, team practice, event or open gym within any FEA facility.
I, hereby grant Force Elite Academy (FEA) permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I understand my child's image, face and/or likeness may be featured in FEA media online via Social media or Website and FEA is not able to completely avoid excluding my child from such media completely.
I understand that I may specifically request for my / my child's name not to be included in FEA social media in writing to [email protected] but understand that FEA may not be able to remove my child's presence from any media altogether.
I understand that FEA uses CCTV camera's for the purpose of security and injury reporting and that all footage is kept and destroyed in accordance to our Privacy Policy
I have read, understand and agree to the terms of this Waiver, Release of Liability & Assumption of Risk and Media Release, and I have signed it voluntarily. If for any reason any provision of this Waiver is found to be invalid, illegal or unenforceable by a court of competent jurisdiction, the validity, legality and enforceability of the remaining provisions shall in no way be affected or impaired thereby. I intend that this Waiver shall constitute a complete and unconditional release of all liability of FEA, its subsidiaries, affiliates, employees and /or volunteers to the greatest extent allowed by law. I hereby hold harmless, release, and forever discharge FEA from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I agree that this Waiver shall be governed by and construed according to the laws of the state South Australia only, and that any lawsuit related to myself and my executors and assigns participation in any activity may only will be held in South Australia.

Booking is per term, and you will be added to our online system. You can pay through there or see reception on arrival. 1hr classes are $120 p/term & 45 min classes are $100 p/term.
